Golf Player Registration
9th Annual HACA Fall Golf Tournament
Date:  Monday, 7 November 2011


The cost to register as a player in the golf tournament on November 7th is $110 for members and $125 for non-members.  Lunch and dinner are included in this price.
We are asking participants to register and make payment by October 31, 2011.

To register for this golf tournament, complete and submit the electronic form below.

This electronic registration form provides for both individual and group registration; register yourself, a foursome, or as many as 12 players.

Please be sure to include each player's HANDICAP.

When you have completed the form, click the "Submit" button at the bottom of the page and follow the prompts to the online payment system.
